Thank you for your interest in exhibiting at the 2009 School Wellness Conference. Please complete the form below for a preliminary review of your organization’s products and/or services.
Application Process:
Final decision will be at the discretion of the conference organizers. Upon approval, you will be sent an Exhibitor reservation form.
Application Fee: $1000
Exhibitor fee includes admittance of two persons to staff your exhibit, exhibitor
badge, one 6’ table, and two chairs. On-site electrical access requests cannot be accommodated. Hanging of company signage on wall prohibited.
Cancellation & Refund Policy:
ExhibitExhibitor fees are non-refundable for no shows or cancellations. Purchase order payment terms are net 30 days from invoice date.
After September 6, full
pre-payment required at time of booking. We reserve the right to cancel your reservation for nonpayment of fees. Direct all cancellation requests via email to register@csba.org.
